Cancellation policy

 


Membership Terms:
All memberships begin as a mandatory 6 month program, Auto-debit charged every 30 days

APPOINTMENT CANCELLATION POLICY: If you need to reschedule your an appointment, please do so 48 hours or more prior to appointment. Failure to give required notice will result in losing the appointment and will be charged $35 late cancellation fee or $50 no-show fee.

MEMBERSHIP FREEZE: Freeze request must be made 15 days before your next billing cycle, unless there is a documented medical issue. Memberships can be frozen up to 3 months for a $50 plan month freeze fee, the only benefits allowed during freeze will be % off products.

MEMBERSHIP CANCELLATION: To cancel, please provide 30 days' written notice before your next billing date for all auto-recurring memberships. Breaking the 6- month agreement will incur an early termination fee of $125. All memberships will continue to be billed unless written notice is provided.  

RETURNED PAYMENTS: all NSF and declined payments will be charged a $35 fee per declined transaction.  

BOOKINGS: All membership plans are on a  biweekly to monthly basis and will not roll over into the next month. Any services not used within the 30 day time allotted will be lost.  

Returns and Refunds: No refunds are provided for services or products. Rescheduling is available if needed.

 

Late Fee:

You are allowed a 5-minute grace period. After 5 minutes, a $20 late charge is due at the beginning of service. After 15 minutes, your appointment is cancelled.

No Call/No Show:
Automatic Cancellation with NO REFUND.

Reschedule:
Must be done 48 hours prior to your appointment.
